Submit a concern without connecting it to a portal account.

This form does not require login and the application does not store your account, IP address, browser, or session with the report.

If you want a private reply, select that option before submitting. You will need to save both the case number and the reply code; the code cannot be recovered later.

Immediate danger: choose Urgent and also notify an available supervisor or emergency service directly. Anonymous portal submission alone is not an emergency response.

Use Urgent for an immediate safety hazard. The report remains anonymous, but the portal cannot replace direct emergency or verbal notification.
